CVP is seeking Long Term Care (LTC) Registered Nurses (RN) to join our team in providing our Nation's Veterans with compassionate, high-quality care at the Houston VA Medical Center's Community Living Center (CLC). The CLC functions as a skilled nursing facility within the VA system, providing care to Veterans with chronic illnesses, disabilities, or those in need of end-of-life care.
The RN will utilize their broad-based clinical knowledge to effectively assess, implement health promotion and prevention interventions, manage acute and chronic illness states, assist patients/residents in the attainment of optimal levels of functioning rehabilitation, evaluate patient/resident response to actual or potential health problems, and provide supportive measures for the patient/resident and their significant others.
Deliver direct patient care including assessments, medication administration, wound care, and monitoring of vital signs
Develop and implement individualized care plans in collaboration with interdisciplinary teams
Support residents with activities of daily living (ADLs), such as bathing, dressing, feeding, and mobility assistance
Monitor changes in residents' physical and mental health and promptly report concerns to the medical team
Provide emotional support to Veterans and their families, particularly in palliative and hospice care settings
Maintain accurate documentation in accordance with VA policies and electronic health record standards
Participate in quality improvement initiatives, infection control, and safety protocols
Perform head-to-toe assessments and vital sign monitoring, including pain assessment
Administer medications safely and accurately using the Bar Code Medication Administration (BCMA) system
Perform wound care including dressing changes and obtaining wound cultures as necessary
Complete Braden Scale assessments to measure risk for skin injuries and implement preventative interventions
Conduct Fall Risk Assessments and implement strategies to decrease the risk of falls and injuries
Clear all CPRS clinical alerts daily and maintain accurate electronic health records
Practice the VA core values: Integrity, Commitment, Advocacy, Respect, and Excellence (ICARE)
Provide clinical oversight to LPNs, Nursing Assistants, and other support staff as needed
Must be eligible to undergo a Government sponsored background investigation (US Citizenship Required)
Bachelor of Science in Nursing (BSN) required - Graduate of a school of professional nursing approved by the appropriate State agency and accredited by either the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
Current, full, active, and unrestricted RN license in any U.S. state or territory
Current American Heart Association (AHA) Basic Life Support (BLS) certification
Minimum of one year of recent experience in Long Term Care (LTC) or Skilled Nursing Facility (SNF) settings
AHA Advanced Cardiovascular Life Support (ACLS) preferred
Experience in hospice/palliative care, geriatrics, or medical-surgical nursing preferred
Familiarity with electronic health records systems, preferably experience with VA's CPRS
