Overview

CVP is seeking Licensed Clinical Social Workers to join our team in providing our Nation's Veterans with high-quality care at the Southern Arizona VA Health Care System (SAVAHCS) in Tucson, AZ. We have openings for both In-Patient and Out-Patient Social Workers.

 

Location: Southern Arizona VA Health Care System (SAVAHCS), 3601 S. 6th Ave. Tucson, AZ 85723

*Out-Patient locations will vary

Work Schedule: Full-time, 40 hours per week. Specific shifts to be determined.

Contract Period: Base year plus four (4) one-year options, starting January 2025

Responsibilities

  • Perform Biopsychosocial Assessments
  • Assess patients and plan appropriately to assist with social determinants of health
  • Conduct Suicide Risk, Mental Health, and Crisis Assessment and intervention
  • Document safety plans and other plans of care in the patient's medical record
  • Provide information and assist with completion of Advanced Directives
  • Assess and complete process for Petition of Inpatient Psychiatric Care
  • Communicate pertinent patient data to providers and interdisciplinary team
  • Reassess the patient's condition and revise plans of care as needed
  • Provide resources and assistance with transportation
  • Schedule and conduct family meetings, interdisciplinary team meetings, and goals of care conversations
  • Provide resources for veterans determined to be homeless
  • Assess and address any findings of abuse or neglect, consulting Adult Protective Services as appropriate
  • Assess and assist with discharge planning, including referrals to various care facilities and home health services
  • Identify veterans who need case management services post-discharge
  • Assess patient/family knowledge level, implement plan of instructions, and record demonstrated level of understanding
  • Able to initiate, prioritize, and reorganize appropriate measures in emergency situations
  • Can initiate life-saving measures in the absence of Physicians
  • Assures the content and correctness of a prepared report and signs the document to validate its content
  • Review all information in the electronic medical records pertinent for social work services
  • Maintain professional decorum and appearance while consistently applying VA's customer service standards
  • Respects patient’s rights, including but not limited to visual and auditory privacy

Qualifications

  • Master of Science in Social Work (MSW) from a nationally accredited school
  • Current, full, and unrestricted license to practice as a Social Worker in any State, Territory, or Commonwealth of the United States, or the District of Columbia
  • Minimum of two years' current experience in Social Work
  • Ability to obtain and maintain VA credentials and privileges (U.S. Citizenship Required)
  • Strong communication and interpersonal skills
  • Proficiency in electronic medical record systems
  • Ability to work effectively in a multidisciplinary team environment

Preferred Qualifications:

  • Prior experience working with Veterans or in a VA setting
  • Familiarity with VA policies and procedures
  • Experience in either in-patient or out-patient settings, as applicable to the position
